CT Imaging in Equine Medicine: How Advanced Diagnostics Change Outcomes

What is CT imaging, and how does it work in horses?

CT stands for computed tomography. The machine takes a rapid series of X-ray images from multiple angles and uses computer software to reconstruct them into detailed, three-dimensional cross-sections of tissue, bone, and soft structure. Think of it as the difference between a flat photograph of a building and a full architectural model. You can rotate it, examine layers, and find problems that a single flat image would miss entirely. 

In horses, CT is particularly powerful for evaluating the head, neck, distal limbs (from the knee or hock down), and foot structures. These are areas where complex anatomy is densely packed and where traditional imaging has historically left clinicians making educated guesses. 

CT imaging vs. traditional X-ray: a patient-friendly comparison

Both tools have their place. X-ray is fast, widely available, and excellent for straightforward bone evaluation and fracture screening. It’s often the right first step. But it’s limited to two dimensions, soft tissue detail is minimal, and overlapping structures can obscure findings. 

CT changes that. It provides three-dimensional cross-sectional views, exceptional detail in both bone and soft tissue, and the ability to detect lesions that flat film misses entirely. When X-rays return inconclusive results, when a horse continues to show pain without a clear source, or when surgery is on the table, CT provides our team with clarity that changes the clinical conversation. 

When we recommend CT over conventional imaging

Not every case calls for CT. Here are the situations where we reach for it most often. 

Complex lameness with no clear origin. Some horses block out of a region beautifully on a nerve block but show nothing convincing on radiographs. CT often reveals small but significant lesions in the navicular bone, the coffin joint, the pastern, or the surrounding soft tissues that standard views can’t capture. 

Head and sinus conditions. Dental disease, sinusitis, and sinus cysts are notoriously tricky to evaluate with flat film because the anatomy of the equine skull is so layered. CT lets us examine individual tooth roots, sinus compartments, and bony margins with remarkable clarity. 

Surgical planning. Whether we’re discussing a fracture repair, a neurectomy, or a more complex orthopedic procedure, a three-dimensional map of the affected area helps us plan more precisely, anticipate complications, and communicate clearly with owners about what surgery will involve. 

Young horses with developmental concerns. Osteochondrosis and other developmental joint conditions sometimes present with subtle changes that radiographs underestimate. Early, accurate diagnosis allows for timely intervention, which often correlates with better long-term outcomes. 

What does a CT scan involve for my horse?

Horses require general anesthesia for CT. That’s a meaningful consideration, and we take it seriously. Our anesthesia protocols are designed specifically for equine patients, and our team manages every aspect of the procedure with their safety as the primary concern. 

The scan itself is typically short. Reconstruction and review of the images takes additional time, but we walk owners through findings as soon as they’re ready. You deserve to understand exactly what we’re seeing and why it matters. 

CT Myelogram Services

A CT myelogram is one of our most valuable tools for diagnosing cervical issues in horses. During this procedure, contrast dye is injected into the space around the spinal cord, allowing us to visualize compression, malformations, or other abnormalities in the neck that standard imaging often misses. This level of detail is essential for horses showing neurologic signs, ataxia, or neck pain with no clear cause. Because precise, motion-free images of the spinal canal are required, CT myelograms are always performed under general anesthesia, unlike some of our other CT scans, which can be done standing. This distinction matters for owners weighing the procedure and its recovery.
